TERMINAL ILEITIS: OVERVIEW

Terminal ileitis (TI) is a chronic inflammatory disease of the gastrointestinal (GI) tract. It is a condition that affects the intestine and involves the terminal part of the ileum (the small intestine). Though the disease mainly pertains to the small and the large intestines, it can occur at any part of the digestive tract --- from the mouth up to the anus. Terminal ileitis is also called Crohn's disease or granulomatous enteritis.

Terminal ileitis basically causes ulcerations (breaks in the mucosal lining) of the small and large intestines, and is characterized by the inflammation of the ileum. Although the exact cause of the occurrence of Terminal ileitis is not known, it is believed that the disease can potentially be a consequence of infection, severe irritation, obstruction, or faulty absorption of substances via the intestinal walls.

In the early stages of Terminal ileitis, small and shallow crater-like erosions called 'aphthous ulcers' erupt, in a scattering way, in the inner part of the bowel. Over a period of time, the condition leads to the development of bigger and deeper ulcers due to which the bowel gets scarred, and becomes stiff and narrowed, thereby resulting in obstruction. The big and deep ulcers can damage the bowel wall, and lead to infection in the peritonitis (abdominal cavity) as well as in the adjoining organs.

Terminal ileitis generally occurs in people aged between 15 and 30 years, and is more common in men as compared to women. The disease is usually chronic; and can recur with phases of remission and aggravation (that is, periods of activity and inactivity).

SYMPTOMS OF TERMINAL ILEITIS

Terminal ileitis typically involves the distal part of ileum, and its main indications are cramping, abdominal pain (acute pain in the right lower quadrant) and diarrhoea. Besides these indications, some of the other common symptoms of the disease are:

  • Vomiting
  • Fever
  • Bloody stools
  • Weight loss
  • Anal fistulae
  • Perirectal abscesses
  • Poor appetite
  • Night sweats
  • Rectal pain
  • Reddish tender skin nodules
  • Inflammation of the spine, joints, and liver

TYPES OF TERMINAL ILEITIS

On the basis of the symptoms, and on the affected portions of the GI tract, Terminal ileitis can be categorized into the following five types:

  1. Ileocolitis: when the condition chiefly affects the ileum (or small intestine) and the colon.
  2. Ileitis: when the condition mainly affects the ileum.
  3. Gastroduodenal Crohn's Disease: when the condition primarily affects the stomach and duodenum (the starting point of small intestine).
  4. Jejunoileitis: when the condition mainly affects the upper half of small intestine.
  5. Granulomatous Colitis: when the condition affects only the colon.

TREATMENT OF TERMINAL ILEITIS

Terminal ileitis is a disease for which there is no specific medical treatment. To provide relief to the patients suffering from this disease, doctors mostly recommend the use of medications for managing inflammation, antibiotics, immune suppression, or a surgery. The condition is generally diagnosed with the help of barium x-ray of the small bowel, barium enema, and colonoscopy.
